Know Your Anal Warts

There are some subjects in medicine which are considered more taboo than others, especially to patients who are otherwise young and sexually active. Genital warts, and particularly anal warts, may be at the top of this list – a painful and embarrassing problem which nonetheless can benefit from candid conversations.

So let’s have one. Recently a patient asked me how to know whether he has anal warts, and how to tell these apart from other lesions, scabs, or hemorrhoids that may appear in the area.

Anal warts appears first as very tiny spots and then grow into larger shapes. Initially, the patient may not be aware of their appearance. But with time, they grow larger and spread into the surrounding areas.

Anal warts tend to be firmer than hemorrhoids, and they spread and grow in a way that hemorrhoids never could. Moreover, because they are considered a precursor to anal cancer, these warts require medical intervention as soon as possible, making early diagnosis a paramount concern.

An Uncomfortably Close Look At Anal Warts

Anal warts can be a source of great discomfort and greater embarrassment, especially for single people who don’t like to make a bad first impression below the belt. Many of my patients are devastated when they are first diagnosed with anal, vaginal or penile warts or condyloma. One of their first concerns are how to notify their previous and future sexual partners of this condition.  Many of my patients experience a wide array of emotional feelings when they are initially diagnosed. To many, this is devastating, however, the good news is that it is a curable condition.

Anal warts are also potentially dangerous, as they are considered a harbinger of several more serious clinical conditions such as anal dysplasia and anal cancer. Caused by the human papilloma virus (HPV), anal warts can spread and change over time, impeding your ability to sit, defecate, and enjoy life comfortably.

Understanding Anal Pain

What causes anal pain? Although this is a common symptom, it doesn’t always have a common cause; a wide array of medical concerns may lead to pain in the anus, and understanding the differences between them is essential to get the treatment you need and not miss a serious cause such as anal cancer and/or colon and rectal cancer.

The most common source of anal pain is a condition known as anal fissure, which is a tear in the wall itself. Anal fissure is an extraordinarily painful condition which tends to remain acute long after bowel movements are complete, greatly interfering with a patient’s quality of life. Typically, 90% of anal fissures occur in the posterior midline (towards the spine) and less than 10 % occur in the anterior midline (towards the genitals). Anal fissures that occur on the left or right lateral wall of the anal canal are more worrisome and should be investigated further for cause such as inflammatory bowel disease, HIV and TB to name a few.

But some other causes can include everything from anal cancer and anal abscess to fistulas, anal warts, and rectal abscess. Additionally, thrombosed external hemorrhoids are also very painful making it very difficult to sit, stand, walk and off course have a bowel movement. Last, prolapsing internal hemorrhoids or internal hemorrhoids that actually prolapse or protrude outside of the anal opening can be very painful and may because strangulated and necrotic, making it a surgical emergency.

A word about the author: My name is Dr. Maziar Ghodsian, but my patients call me Dr. Maz. I am one of just two proctologic surgeons in the state of California that has completed a residency in Proctology or Proctologic Surgery. I trained at UC Riverside and Johns Hopkins, and completed a residency in Proctologic Surgery at Ohio University-Grandview Medical Center/Kettering Health Network. I also spent a portion of my time during residency at Cedars Sinai Medical Center and the Cleveland Clinic, training with world renowned surgeons.
